Children under the age of 18
Children under the age of 18 must be accompanied by an adult. If someone other than the parent/guardian is to accompany the child, the parent/guardian must send a
notarized
letter indicating this consent.
No Shows
If you must cancel or reschedule your appointment with one of our physicians or the physician assistant, please provide 24 hour notice. Our office does hold the right to charge $25.00 for any missed appointments.
Due to the length of time involved and long waiting lists, if you must cancel or reschedule an appointment for specific testing such as allergy testing or vestibular testing, 48 hours notice is required. There is a $100.00 fee for appointments not cancelled or rescheduled within the 48 hour time frame.
